A monthly payment of $99 entitles you to the following benefits:

Additional Membership Rules:
- Offers can’t be combined with promotional offers, non-transferable and non-refundable. Limit of 20 units of BOTOX® per quarter and on the first 20 units.
- Member agrees to a minimum 6 month commitment which will be billed monthly.
- Membership may be cancelled at any time after the initial 6 month commitment.
- Monthly treatments must be used within the allotted 30 days or they will not be rolled over into the following month.
- Any treatments that are not redeemed will be lost.
- Memberships must be cancelled in person or via email at info@suitesixmedspa.com. Please provide your name, phone number, email address and date you'd wish to terminate your membership.
*This monthly membership fee recurs monthly around the date of purchase unless cancelled by following the steps above. To receive discounted pricing online during your membership, you must be logged into your account on the store and follow the instructions during checkout. Monthly service options are only available in the office.
